By completing this form I am certifying that all of the below information is correct to my knowledge. I am also giving consent for the CARPOOL advisor to check my grades. I also understand that if I am selected as a CARPOOL member and I do not have a 2.0 GPR for Spring 2008 and cumulative, I will be removed from membership. I also understand that as a member of CARPOOL, I will be expected to pay $35 dues at the beginning of each semester and fulfill all requirements of membership.
